Update Readme.md installation command to @modelcontextprotocol/sdk

Update readme to replace npm install @modelcontextprotocol/server zod and npm install @modelcontextprotocol/client zod with npm install @modelcontextprotocol/sdk zod

Motivation and Context

The package now lives at https://www.npmjs.com/package/@modelcontextprotocol/sdk, and running npm install @modelcontextprotocol/server and npm install @modelcontextprotocol/client returns an npm Not Found error.

Hello, main now holds the v2 alpha version of the SDK, thus the package name changes - however, it's not published yet.

Please find the v1 as per instructions in the main README.md
